HealthCheckPlus contains are range of features that allow clients to customize the types of surveys and utilize the program for a variety of objectives. Our applications were designed to meet the security requirements of health researchers in hospital and academic settings.

Customizable:
This system currently screens for a variety of domains, but can be configured to assess any number or combination of domains, using exisiting or custombuild screening tools. Current modules include: Mobility & Exercise, Sleep, Depression, Anxiety, Self Harm, Suicide, Alcohol Use, Drug Use, Gambling, Smoking, Aggression towards others, Bullying & Harassment in the Workplace, Eating Disorders, Sexually transmitted Infections, Pain).
Assessing Patient Care Experiences
The system can be used evaluate a range of patient care experiences (e.g., access, quality of care, stigma, etc.) either on-site or post-visit through a secure link to a follow-up questionnaire.
Multi-lingual capacity
Meeting the health care needs of new non-English speaking Canadians may be difficult. The tools are currently offered in English and French, but can be administered in any language including Arabic. The last census showed that 1 in 6 Canadians was born in a different country.
Quarterly Summary Reports
Quarterly reports summarizing the difficulties of patients reported in your clinic can be easily generated by age, gender, or treating physician as required.
Facilitate Self Study
The system can be used to assist physicians in conducting a self-study of their practice and clinic.
Flexible
The system has a flexible infrastructure that can be programmed to implement any number of screening modules, add new modules, modify existing modules, or target specific age or demographic groups.
Knowledge Mobilization
The system can be used to direct relevant information to providers and patients as needed and on-demand. Centralize dissemination ensures that all providers will have access to the same information, updates and advisories. In a survey of 1878 Canadian physicians, only 52% reported that they used guidelines at least once a month, and more than 25% expressed concerns about the source of the guidelines.
Multiplatform:
Both programs can be implemented over the internet, PDAs or smart phones.
Clinic "Kiosk" requirements
Requirements are minimal and include (a) space to house a computer kiosk with a touch screen monitor (3'x2'x4' WxDxH), (b) access to the internet, (c) access to a printer (networked or stand alone), (d) start the computer each day, (e) launch a web-browser and logon (once) to the secure system.
Upgrades and Technical Support
All end users are entitled to all upgrades, improvements, and revisions instantaneously (by virtue of this being an internet-based system).

FDA Compliance
Design and data collection applications are compliant with the US FDA regulations on collecting and maintaining Electronic Records (i.e., Part 11 of Title 21 of the Code of Federal Regulations regarding Electronic Records and Electronic Signatures).

Data Backup
Data and applications are backed up nightly to a local electronic medium (mirrored IDE Hard Drives). Data and applications are backed up weekly to a remote and removable? electronic medium (CD-ROM) which are stored in a physical location different from the physical location of our secure servers. Access to the electronic medium is restricted to a dedicated office at iSurvey Canada
Software
Applications are implemented in PHP running under an enterprise version of Linux.
Security upgrades and patches are implemented in a timely manner.
Hardware
Applications are hosted on dedicated IBM or Dell servers, which do not simultaneously run e-mail applications, which are vulnerable to software viruses designed to compromise servers, applications and databases.
Servers are rack mounted and located in secure location with 24 hour supervision and security.
Access
Access to the databases and software applications is restricted to just three key individuals (iSurvey CEO, the Senior Software Developer, and the Network Support Specialist).
Repeat access to surveys can be restricted
Participants can be limited to taking the survey just once
- automatic timeouts disable access to both administrators and survey respondents after long period of inactivity (i.e.,more than 30 to 60 minutes).
Accounts that are not accessed for more than 30 days are automatically deactivated.
Individual studies can be immediately suspended, if and when required, without deleting or resetting individual accounts or suspending other studies.
Data Transmission
Data being submitted or downloaded from our servers are transmitted over a secure socket layer using 128-bit encryption. This includes responses to surveys, creation and editing of all surveys, databases or accounts, as well as downloading data files.
Privacy
Patient accounts can be created without using personal identifying information but still integrated with secure electronic chart systems.
